Volunteer Requirements:

 

Kiski Area School District requires all program volunteers to submit the following three (3) clearances:

(PLEASE NOTE: The FBI Fingerprint Clearance is ONLY required if you have been a resident of PA for less than 10 years.  If you have been a resident for 10 years or more, THIS affidavit can be completed.  Please send this affidavit in with your child in an envelope marked PTO membership or email it to kiskinorthpto@gmail.com with your name/clearances in the subject line.  This affidavit needs to be send in each year regardless if your other clearances are still valid.)

 

***PA State Police Criminal Record Check and PA Child Abuse History Clearance will need to be updated every 5 years***

***The PTO Affidavit needs to be printed/signed/submitted EVERY YEAR***

 

All volunteers clearance forms need be on file with the school prior to signing up for volunteer opportunities.  Please send a copy of your clearances/affidavit to Kiski North PTO Membership in order to aid in this process. Clearances/affidavit can be sent in with your child in an envelope marked PTO Membership or emailed to kiskinorthpto@gmail.com.  Please include your name/clearances in the subject line.  Thank you!
